New Patient Registration
Elm House Surgery welcomes new patients.
If you need to register with a doctor and are living in the BR3 catchment area, you may register with us.
Please visit the Surgery to collect your registration documents. Alternatively you may print off the registration form and new patient health questionnaire using the links below.

Elm House - Under 5's additional information
Please note - the is a separate health questionnaire for under 5's as it is important we obtain full immunisation history prior to registration.
The registration form and health questionnaire must be completed in full and then handed in at reception. A relative or friend can fill in the form with your consent.
Please note, to process your registration, all forms need to be completed. You will also need to produce two forms of Identification, 1 to be photographic.
Some examples of they type of identification documentation are:
- Driving Licences
- Utility bill
- Phone bill
- Bank card / Bank Statement
- Rent book or tenancy agreement
- Council tax bill / Council rent book
Where patients are registering to join the practice, the practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.
