How to Register

Elm House Surgery welcomes new patients.

If you need to register with a doctor and are living in the BR3 catchment area, you may register with us.

Please visit the Surgery to collect your registration documents. Alternatively you may print off the registration form and new patient health questionnaire using the links below.

The registration form must be completed in full and then handed in at reception. A relative or friend can fill in the form with your consent.

While your registration is being processed we need to confirm that you are who you say you are. Please bring along with you two forms of identification, one to be photographic.

Some examples of the type of identification documentation are:

  • Passport
  • Driving Licences
  • Utility Bill
  • Phone Bill
  • Bank Card / Bank Statement
  • Rent book or tenancy agreement
  • Council Tax bill / Council rent book

Once you have completed all the necessary documentation, your registration will be processed by our administrative team.
